You Cannot Fix What You Cannot See — Community Health Diagnosis Saves Lives
Across rural western Uganda, communities carry invisible health burdens — chronic disease that goes undiagnosed, children who have never been vaccinated, mothers who have never attended a prenatal clinic. These realities exist in silence until someone shows up, asks the right questions, and records the truth.

Why Community Diagnosis Matters
In global health, the most dangerous assumption is that we already know what communities need. Too many programs deliver the wrong solutions to the wrong places because no one took the time to ask. HAFT's community diagnosis program ensures that every intervention we run is driven by real, locally gathered evidence.
In the communities we serve, we have found:
- Families living with undiagnosed diabetes, hypertension, and malnutrition — conditions that are treatable when identified early
- Children who have missed critical vaccinations because no one tracked their immunization history
- Households facing food insecurity, sanitation crises, and waterborne disease that external health workers never knew existed
- Elderly and disabled community members completely isolated from all health services
How Our Teams Work Right Now
Our trained volunteer health workers are currently conducting systematic household visits across our target communities. Each team:
- Maps every household in a village, recording family composition, ages, and health status
- Identifies households with pregnant mothers, infants, elderly members, and persons with disabilities
- Documents current illnesses, recent deaths, and chronic conditions
- Records immunization status for all children under five
- Flags urgent cases for immediate referral and follow-up
This data does not sit on a shelf. It drives our program planning in real time — determining where we deploy our maternal health teams, which children need immunization follow-up, and which households need nutritional support most urgently.
